Amending Information
    • Patient/relative informs nurse of an incorrect information such as patient name, birthday, etc.,
    • Instruct relative to go to the Admitting Section for Amendment of information.
    • Admitting will process the amendment and will instruct the patient to present the Amendment for to the NOD to be attached in the patient chart.
    • Patient/relative informs nurse of an incorrect information such as patient name, birthday, etc.,
    • Instruct relative to go to the Medical Record Section for Amendment of information.
Patient’s Watchers
    • Fully Vaccinated
    • 22-59 y/o
    • Present Vacination Card and ID
    • For Partial or Unvaccinated -Mandatory Antigen Testing
    • Fully Vaccinated

Kinds of Admission

      Direct Admission

      - with referral from the Attending Physician or Physician of Choice of the patient.

    • ER nurse will instruct the relative to proceed to the Admitting Office for priming.

      ER Admission

      – patients who were initially for outpatient consultation only from the ER and eventually gets admitted.

    • ER nurse will enter the patient details in the ER module and asks relative to proceed to Admitting Office for priming.
